To describe racial and income disparities in maternal diabetes incidence over time in the United States. We used US vital statistics data for 1986 to 2012. Incidence of maternal diabetes was estimated using age- and ethnicity-specific birth rates. We used fixed-effects models to estimate incidence trends, and Poisson models to describe disparities in incidence rates. Births were estimated to be 157,382,052 (11.4%) to 168,029,821 (12.5%) for the overall population. Among births with diabetes, 37% were to Hispanic, 21% were to black, 16% were to white, and 8% were to Asian women. The rate of diabetes was highest in births to black women (5.08 per 100,000; 95% CI, 5.02-5.13), followed by births to Asian women (3.41; 95% CI, 3.36-3.45), Hispanic women (3.11; 95% CI, 3.08-3.15), and white women (2.85; 95% CI, 2.81-2.90). During the time period, diabetes incidence decreased for all race and ethnicity groups. Incidence was highest in the South (5.61; 95% CI, 5.51-5.70) and lowest in the West (3.13; 95% CI, 3.06-3.21).
